Immunization Regulations

Please understand that the state of Colorado now REQUIRES children attending Colorado schools to be immunized. Be sure to contact your child’s doctor if you have not already done so to acquire a signed copy of your child’s immunization records. Once you have that copy, you may transcribe those immunizations onto the state form to have ready for Parent Orientation night. We cannot allow your child to attend the preschool until this form is completed. If you do not have access to a printer, we will have copies of this form available at orientation.
